SUBJECT:

Consider motion to adopt RESOLUTION authorizing the City of Ankeny, Iowa to execute a death claim form for release of life insurance proceeds in accordance with Resolution 214-96.
EXECUTIVE SUMMARY:
On December 19, 1977, the City entered into a lease agreement with Robert Spooner for the lease of certain property within the City.  The lease provided that the City would obtain a life insurance policy with a death benefit of $100,000 payable to the person designated by Mr. Spooner.  Resolution 214-96 dated August 19, 1996, designated James L. Moyer as the beneficiary.  Robert Spooner died on December 5, 2016 and the insurance carrier has requested that the City execute a death claim form as part of the documentation required to release the funds to the designated beneficiary.
